Icon too big on home screen

I have a customer that on the home screen all the icons are large only one shows at a time how do I change this?

It is Apple berry classic now but did it on standard zen. It works on mine fine. I have never had this one big icon.

Have you tried uninstalling the theme and then reinstalling it, also what OS is the device using?

Options -> Advanced Options -> Accessibility -> Grid Layout
